I have a few suggestions for you. First you may want to have the meeting with your supervisor and Instructional Specialist at the same time. It will save you time and I also think it will build a sense of trust and teamwork between your supervisor, Inst. Spec., and yourself.
Also activity 3 states you will discuss the need for the Mentor Academy...you may want to specify who you will be discussing this with. You may also want to run this by the SBDM because it seems like this is a project that will impact the whole school.
I like the fact that you will be evaluating the progress of the project periodically and not just at the end of its implementation. Good Work!
